Raffle Forms

The cost of each ticket is $1.00 or 6 for $5.00. If you would like to purchase a ticket or tickets, please fill out ticket information below and send money and ticket(s) in an envelope marked “5k Fun Run Raffle”. Please have tickets in by Friday, Oct. 14 at 12:00 Noon. Cash or checks payable to your schools PTA. Tickets will also be sold at the 5k Fun Run on Saturday until 10:00 AM. Drawing will be held at the 5k Fun Run on Saturday, October 15. (Attendance not required to win).
